摘要
选取2018年7月-2020年10月该院106例OSAHS患者作为研究对象,均于清醒状态下行苗勒氏试验及药物诱导睡眠内镜检查,记录检查结果。对比患者不同状态下上气道阻塞平面形态和上气道腭咽、口咽侧壁、舌根和会厌(VOTE)评分,并分析VOTE评分与呼吸暂停低通气指数(AHI)、血氧饱和度的关联性。
106例患者气道阻塞形态分布:腭咽平面以环形阻塞为主,完全阻塞、部分阻塞分别占84.91%和7.55%;其次为前后阻塞,完全阻塞占4.72%;口咽侧壁平面均为左右阻塞,完全阻塞、部分阻塞分别占60.38%和16.98%;舌根平面均为前后阻塞,完全阻塞、部分阻塞分别占16.98%和22.64%;会厌平面均为左右阻塞,无完全阻塞,部分阻塞占比15.09%。清醒状态下上气道阻塞形态分布:腭咽平面均为环形阻塞,完全阻塞、部分阻塞分别占52.83%和39.62%;口咽侧壁平面均为左右阻塞,完全阻塞、部分阻塞分别占30.19%和24.53%;舌根平面均为前后阻塞,无完全阻塞,部分阻塞占4.72%;会厌平面均无阻塞。睡眠状态下检查阻塞平面数多于清醒状态,各平面阻塞程度评分均高于清醒状态(P < 0.05)。OSAHS患者睡眠状态下阻塞平面数、各平面阻塞程度均与AHI呈正相关,与平均及最低血氧饱和度呈负相关(P < 0.05)。
阻塞型睡眠呼吸暂停低通气综合征(obstructive sleep apnea hypopnea syndrome,OSAHS)是一种睡眠呼吸疾病,外科手术是治疗的重要方法,但手术方法的选择需根据患者病因,否则难以获得理想的疗
选取2018年7月-2020年10月本院106例OSAHS患者作为研究对象。其中,男73例,女33例;年龄18~60岁,平均(38.24±10.11)岁;体重指数(body mass index,BMI)18.4~28.5 kg/
检查前30 min,采用局部麻药棉片及喷剂(1%丁卡因)行鼻腔表面麻醉,患者在清醒状态下去枕仰卧,指导患者口鼻闭紧做奋力吸气动作,模拟睡眠状态,将奥林巴斯OTV-S7型电子鼻咽喉镜置于鼻后孔稍下方,根据VOTE评分标
腭咽平面以环形阻塞为主,其中完全阻塞90例(84.91%)(
患者清醒状态时可见腭咽平面为环形阻塞,未见前后及左右阻塞,其中完全阻塞56例(52.83%)(

睡眠状态下检查阻塞平面数多于清醒状态。患者在清醒和睡眠状态下不同平面数比较,差异有统计学意义(P < 0.05)。见

睡眠状态下会厌阻塞程度评分为(0.15±0.02)分,清醒状态下为0分。睡眠状态下各平面阻塞程度评分均高于清醒状态,两者比较,差异有统计学意义(P < 0.05)。见
患者在睡眠状态下,随着阻塞平面数增加,AHI明显升高,平均及最低血氧饱和度明显下降(P < 0.05)。见
上气道狭窄和阻塞是OSAHS的直接发病机制,但其发病并非简单的气道阻塞,而是上气道塌陷,并伴有呼吸中枢神经调节因素障碍,主要症状为夜间睡眠打鼾伴呼吸暂停、白天嗜睡,可引起多系统功能异
既往多采用清醒状态下苗勒氏试验对OSAHS患者上气道阻塞平面及塌陷方式进行评估,此项检查在患者清醒状态下进行,易被神经反射、肌肉代偿作用影响,评估效果不理想,尤其是受颏舌肌代偿收缩作用的影响,难以观察舌根松弛后坠现象和咽侧壁塌
综上所述,药物诱导睡眠有助于改善内镜检查对OSAHS患者上气道阻塞形态的评估效果,且睡眠状态下阻塞平面数、各平面阻塞程度均与AHI和血氧饱和度密切相关。药物诱导睡眠内镜检查具有实时、动态的观察优势,但需要注意的是:该检查方法受药物作用的限制,检查时间不能过长,且药物诱导的睡眠与整夜的自然睡眠仍存在一定差别,药物诱导睡眠的深度如何控制也缺乏统一标准,仍需通过大量临床研究深入探讨。
